You will need your DEA Number, National Provider ID, Professional License Number, License Type, any Healthcare Specialties, Personal & Employer Information, and if you are a Delegate, for whom.

Register with the Pennsylvania Department of Health Medical Marijuana Program.

Certification

Physicians who register with the DOH Physician Registry will have their physician’s license validated by the Department of State, and be prompted to complete a 4-hour training from an approved provider.

The 4-hour Continuing Medical Education training will count towards the physician licensure requirement.

Register with the Minnesota Department of Health Medical Cannabis Registry

Minnesota has three types of practitioners that can certify a patient with a qualifying condition for medical cannabis:

  • Minnesota licensed Doctor of Medicine
  • Minnesota licensed Physician Assistant acting within the scope of authorized practice
  • Minnesota licensed Advanced Practice Registered Nurse who has the primary responsibility for the care and treatment of the qualifying medical condition of a person diagnosed with a qualifying medical condition
